A handpiece drives a grid of fine needles to a depth the provider chooses, usually between about 1 and 4 mm, then sends radiofrequency energy through them for a fraction of a second. That leaves tiny heated zones at a controlled depth, with untreated skin between. Most devices use insulated needles, where only the tip conducts energy, so the heat lands in the dermis and the surface is largely spared. Some older devices use uninsulated needles, which heat along the whole shaft including the epidermis. That distinction matters more than the brand name, particularly on deeper skin tones. Morpheus8 is one device among several. Profound, Vivace, Genius, Secret RF, Scarlet and Sylfirm X do the same job with different needle counts, depths and energy delivery. The brand is marketed heavily; the settings and the operator determine the result.
How It Works

Two injuries happen at once. The needles make channels, which trigger the same collagen-building repair as plain microneedling. Radiofrequency current then meets resistance in the tissue around the needle tips, generating heat exactly where the tips sit.
The heat shrinks existing collagen fibers right away — a small tightening that shows early and is mostly temporary. Over the following months it drives a stronger, longer remodeling response than needling alone, laying down new collagen and elastin at the treated depth. Deeper settings also reach the fibrous layer under the skin, where any change in jawline definition comes from.
For indented acne scars, the extra depth and stimulus build tissue under the scar floor, with more force than plain needling.
What matters for skin tone is what the energy targets. Lasers are absorbed by pigment, so melanin heats whether or not it was the target. Radiofrequency is absorbed by tissue resistance, and melanin does not change that. It is not colorblind — heat can still trigger pigment — but the mechanism removes the biggest risk.

Redness, swelling, pinpoint scabs and a temporary grid are expected. The problems worth knowing come from heat placed too deep, too hot, or in the wrong face.
In Deeper Skin Tones
−
This is the main reason radiofrequency microneedling exists as a category. Ablative lasers deliver light that melanin absorbs, so on deeper skin tones some of the energy always heats pigment that was never the target — which is why pigment problems after CO2 resurfacing are common enough to change who is offered it. Radiofrequency heats tissue by electrical resistance, and melanin does not increase that resistance, so the energy delivered does not rise with skin color.
That makes the treatment usable on deeper skin tones, not risk-free. Heat in the dermis causes inflammation, and inflammation triggers post-inflammatory hyperpigmentation — flat brown marks a few weeks later. Here they sometimes follow the device's grid pattern, more noticeable than diffuse pigment while it lasts. It fades over months with sunscreen and a pigment cream.
Two things are worth asking about. Insulated needles deliver energy only from the tip, sparing the epidermis where most pigment sits; uninsulated needles heat the whole shaft. Ask which the device uses. Then ask about energy and depth on the first session — a provider used to deeper skin tones starts lower and builds up rather than treating at maximum on day one, and spaces sessions at least four to six weeks apart.
Most also use a pigment-blocking cream for two to four weeks before and after, and are firm about daily sunscreen for a month; sun on freshly heated skin turns an avoidable risk into a visible one. If you have ever formed a keloid or thickened scar, say so before the first session — it changes how deep a careful provider will go.

Stopping after a full course does not undo anything. Collagen built into a scar floor stays there, and texture improvements hold.
Firmness is different. Any tightening competes with ongoing aging, so it fades over roughly one to two years and the face returns toward where it was heading. That is time continuing, not the treatment failing, and it is why maintenance is offered.
Stopping partway through a course is the poor outcome. One session of three delivers a fraction of the result for a large fraction of the cost, since the change is cumulative. If budget is tight, space sessions further apart and finish the course rather than paying for one and stopping.

Finding a Provider

This is a medical device delivering energy into the dermis, so it should be done by a physician, physician assistant or nurse practitioner, or someone they supervise directly. The settings are the entire treatment, which is why who holds the handpiece matters more here than with plain needling.
Useful questions: which device, whether the needles are insulated, what depth and energy for each area, how many sessions they expect, and how many people with your skin tone they have treated. Ask what they do about a dark mark, and how they avoid fat loss in the lower face. A provider who has thought about that will answer quickly.
Red flags: before-and-after photos suggesting a facelift result, a package sold before an examination, no discussion of settings, treating at maximum depth on a first visit, and no mention of sun protection or pigment risk.

- "Morpheus8 is a non-surgical facelift." It is not. The firming is modest and suits early, mild laxity. Skin that hangs needs surgery, and being sold this instead is the most common source of disappointment.
- "It is the same as microneedling, just better." A different treatment. The needles are the delivery route; the heat does the extra work. That brings extra results and extra risks, including burns and fat loss, which plain needling does not carry.
- "Radiofrequency is completely safe on deeper skin tones." Safer than ablative lasers, because melanin does not absorb the energy. Not risk-free — heat causes inflammation and inflammation causes dark marks. Settings and spacing matter.
- "The brand of device determines the result." Several devices do this job. Insulated versus uninsulated needles, the depth, the energy and the operator matter far more than the name on the machine.
- "More sessions always means more tightening." The tightening plateaus. Beyond a sensible course, extra deep sessions add cost, and at high settings in a thin face they add the risk of losing volume.
- "Swelling on day two means it went well." Swelling reflects how much heat went in, not the outcome. It also makes skin look temporarily firmer, which is why people are happiest at day three and puzzled at week three.

Does it work on acne scars?
Partly, and over a course. Rolling and boxcar scars become shallower across about three sessions. Icepick scars respond poorly. Expect improvement, not clearance.
Will it tighten my jawline?
Modestly, if the laxity is mild and early. It firms rather than lifts. Skin that visibly hangs will not respond, and a surgical consultation is the more honest next step.
How much does it hurt?
More than plain microneedling. Numbing cream alone leaves a sharp hot sting, so many providers add injected anesthetic or nerve blocks at deeper settings. With those it is mostly pressure and heat.
Is it safe on deeper skin tones?
Generally safer than ablative lasers, since melanin does not absorb the energy. Dark marks are still possible from the heat, sometimes in the grid pattern, and they fade over months. Ask about insulated needles and conservative starting settings.
Can it cause fat loss in the face?
Yes, and it is the risk worth understanding. Deep, high-energy settings in a thin face can shrink the fat under the skin, leaving a hollow that does not reliably return. Conservative settings built up over sessions, and a provider who takes the question seriously, are the protection.
