Cetaphil Gentle Skin Cleanser is a creamy, low-lather wash for face and body, about $10 to $14 at any drugstore. It is a mild cleanser and nothing else. It does not exfoliate, it does not treat acne, and it will not take off sunscreen or makeup.

The list is ten items long. The surfactant that lifts oil off skin is sodium cocoyl isethionate, one of the mildest in common use, and it sits low on it. Glycerin and panthenol hold water in, and niacinamide came in when the formula quietly changed around 2021. That is why it barely foams and does not leave skin tight.

A cleanser works with surfactants, molecules that grip oil at one end and water at the other, so the rinse water carries oil, dirt and sunscreen away. A surfactant cannot tell the oil on your skin from the fats holding your skin together, so a strong one takes both. That is what tight, squeaky skin is.

It is built to take as little as it can — sodium cocoyl isethionate, one of the mildest surfactants available, at a low level, with glycerin and panthenol left to hold water. The same design explains both habits: it barely lathers, and it does not remove sunscreen.

The 2021 reformulation was an improvement, even if it is remembered as a betrayal. Out went sodium lauryl sulfate, three parabens and propylene glycol; in came the milder surfactant, niacinamide and panthenol. The version people miss was harsher than today's.

The living layer underneath, holding the blood vessels, the nerves and the collagen. It is where lasting change happens and it is hard to reach: most of what is sold for the skin never gets this far.
