Dutasteride: A stronger relative of finasteride that blocks both forms of the enzyme making DHT. In the United States it is used off-label for hair loss, and it stays in the body for months. Oral Minoxidil: The same drug as the scalp liquid, taken as a tablet at a small fraction of the blood pressure dose. It is used off-label for hair loss and needs a prescriber who is watching.

Choose for male pattern hair loss when finasteride hasn't done enough, accepting it stays in the body for months.
Choose for pattern hair loss when the scalp liquid is messy, irritating or forgotten — with a prescriber watching.
Can you take them together?
A common hair-loss pairing: dutasteride slows the hair loss while oral minoxidil helps regrow hair. They work in different ways.
Compare uses by strength of evidence. Evidence level describes confidence in a benefit, not its size.

Blocks two enzyme types: Inhibits types 1 and 2 of 5-alpha reductase. Reduces DHT: Less testosterone is converted into this hair-loss hormone. Helps preserve scalp hair: Reduces follicle miniaturization and can improve hair growth.
Dutasteride inhibits type 1 and type 2 5-alpha reductase and does not bind androgen receptors. Hair-loss use is off-label in the US. Follicles are conceptual, not a guaranteed before-and-after response.

Converted to an active form: The body converts minoxidil into minoxidil sulfate. May open potassium channels: This may alter growth signals in follicle cells. Favors the growing phase: Helps follicles spend more time making hair. Can improve hair density: Some hairs become thicker with continued treatment.
The full hair-growth mechanism is not established. Activation by sulfation is supported; potassium-channel involvement is based on experimental human follicle evidence and is qualified. Oral hair-loss use is off-label in the US. This diagram does not equate vasodilation alone with hair growth.
The long half-life shapes everything. Levels build over weeks and settle after a few months, so nothing here is judged quickly.
Starting dose: 0.5 mg once a day — the dose used in the hair loss trials and approved for hair where it is licensed.
Alternative schedules: Some prescribers use 0.5 mg two or three times a week, since the drug is still present between doses. Lower exposure, less evidence.
Going up: Nothing above 0.5 mg for this use. Higher doses are not a hair strategy.
Total course: Open-ended. The effect lasts only while you take it, with a long tail after stopping.
There is no licensed hair loss dose; this is the range in the dermatology literature. Prescribers start low, go up slowly, and stop where the balance is good.
Starting dose: Usually between a quarter of a milligram and 1.25 mg once a day, lower in women than men.
Going up: In small steps over weeks to months, guided by body hair, ankle swelling and blood pressure, not a schedule.
Usual dose: Broadly 1.25 mg to 2.5 mg a day in women, 2.5 mg to 5 mg in men, though plenty do well below that.
Total course: Open-ended. It works only while you take it.
Any time of day, with or without food. Swallow the capsule whole — do not chew, open or squeeze it, and do not use one that is leaking. Wash your hands if you touch a damaged capsule.
Any time of day, with or without food, but the same time each day. Some take it at night because lightheadedness is easier lying down, especially in the first weeks and after a dose increase. Stand up slowly early on, and split tablets with a cutter rather than by hand.
Take it when you remember, or skip it. Never double up. Because the drug lasts weeks, a missed capsule barely changes your levels — a real advantage over finasteride.
Take it when you remember, unless the next one is close. Never double up. If you have missed several days, ask before restarting at your old dose — tolerance to the blood pressure effect fades.
No routine blood tests are needed for a healthy man taking it for hair loss. Monitoring is a review and photographs, as with finasteride.
Two practical points. Dutasteride roughly halves PSA, the prostate blood test, so any doctor ordering one needs to know. Blood donation is deferred for six months after the last capsule, not one month, because it clears so slowly. Nothing accumulates harmfully; the question is whether to continue, not what to test.
Monitoring is clinical rather than laboratory. Blood pressure is checked at the start and after dose increases, and a doctor will ask at each review about ankle swelling, weight gain from fluid, breathlessness, palpitations and chest discomfort.
Routine ECGs, echocardiograms and blood tests are not standard for a healthy person on a low dose, but they may be requested for anyone with heart or kidney disease, and some prescribers want a cardiologist's opinion first. The high-dose label carries a boxed warning about fluid around the heart and worsening angina — described at doses far above the hair loss range, but the reason this drug is prescribed and reviewed rather than issued.
Approximate windows, not promises.
Months 1 to 3Starting: Nothing visible yet; levels are still building. Some men shed more early as weak hairs are replaced.
Months 1 to 3Extra hair: Fine hair appears on the face, arms or back in a good share of people. The price of the route, not a sign of trouble.
Months 3 to 6Turning: Shedding slows, and hair feels denser before it looks denser. Sexual side effects, if they happen, show up here.
Months 3 to 6Regrowth: First scalp regrowth, usually at the crown and mid-scalp, fine at first and darkening later. Diffuse thinning in women is a common reason it is prescribed, with good reported response — though from case series, not placebo-controlled trials.
Months 6 to 12Regrowth: Regrowth appears if it is going to, mostly at the crown and mid-scalp, where dutasteride beat finasteride head to head. The hairline responds too, but stays the hardest area.
Months 6 to 12Judge it here: Density that has arrived by now tends to hold at a plateau while the dose continues.
After 12 monthsJudge it here: Holding what you have is a result. The deeper DHT suppression gives a modest gain over finasteride, not a different category.
If you stopSlowly backwards: Levels take months to fall, so neither benefit nor side effects disappear quickly.
Weeks 1 to 4Starting: Nothing in the mirror. Lightheadedness on standing up quickly, if it happens, belongs here and usually settles. Ankles may feel tight by evening.
Weeks 2 to 8The early shed: Resting hairs are released so new ones can start, the same as with the topical. Expected, and it settles.
Sorted by what to do about them.
Generic dutasteride is cheap, though it costs more than generic finasteride. Branded Avodart costs far more for the same drug.
Hair loss use is off-label in the United States, so insurance rarely covers it and most people pay cash. Compounded topical and injectable versions cost much more on far less evidence.
Generic minoxidil tablets are inexpensive, and a 2.5 mg tablet split into quarters makes a month's supply cost very little.
Compounded low-dose liquid costs considerably more, because compounding is billed separately. Coverage varies and off-label use for hair loss often is not covered, so many people pay cash. A pill cutter is a few dollars and worth buying.
At 0.5 mg it regrew somewhat more hair than finasteride at 1 mg. Better is not first choice, though — off-label in the United States, and far slower to clear.
Weeks for most of it, traces for months. That is why donation is deferred six months rather than one.
That is how most people start it — a planned switch, not an addition.
Not routinely if you are healthy. Anyone ordering a PSA needs to know, because the result runs at about half.
Intact capsules are fine to handle. Nobody pregnant or who could become pregnant should touch a leaking or damaged one.
DHT recovers over months, not days, and the hair being held is lost over the following six to twelve months.
The same molecule, at a fraction of the blood pressure dose.
Fine extra hair on the face, arms or back is common and dose-related. It fades over months if you stop or reduce.
Two reasons: the liquid is twice a day forever and many people cannot keep it up, and some follicles do not convert it into its active form.
Not routinely if you are healthy. Heart or kidney disease means a fuller assessment, possibly a cardiologist's opinion.
Common, and decided case by case. Tell whoever prescribes it what else you are using.
Some people notice it, usually at higher doses and early on. Your prescriber will ask at each review.
