Dutasteride vs Finasteride

Dutasteride: A stronger relative of finasteride that blocks both forms of the enzyme making DHT. In the United States it is used off-label for hair loss, and it stays in the body for months. Finasteride: A daily tablet that lowers DHT, the hormone behind male pattern hair loss. It is better at keeping the hair you still have than at bringing back hair that is gone.

Which one is right for you?

Choose Dutasteride if…

Choose for male pattern hair loss when finasteride hasn't done enough, accepting it stays in the body for months.

  • Finasteride slowed but didn't hold hair loss
  • You want a once-daily tablet
  • You won't be donating blood soon

Choose Finasteride if…

Choose to keep the hair you still have in male pattern hair loss — the best-studied tablet for it.

  • Thinning at the crown or hairline
  • You'll take it daily for at least a year to judge it
  • You want a cheap, once-daily tablet
Don't combine

Can you take them together?

Both block the same hormone pathway. Taking both adds side effects without clear extra benefit, so choose one.

How it works

Dutasteride

Dutasteride. Lowers DHT to help protect scalp hair. 1. Blocks two enzyme types: Inhibits types 1 and 2 of 5-alpha reductase. 2. Reduces DHT: Less testosterone is converted into this hair-loss hormone. 3. Helps preserve scalp hair: Reduces follicle miniaturization and can improve hair growth.

Blocks two enzyme types: Inhibits types 1 and 2 of 5-alpha reductase. Reduces DHT: Less testosterone is converted into this hair-loss hormone. Helps preserve scalp hair: Reduces follicle miniaturization and can improve hair growth.

Dutasteride inhibits type 1 and type 2 5-alpha reductase and does not bind androgen receptors. Hair-loss use is off-label in the US. Follicles are conceptual, not a guaranteed before-and-after response.

Finasteride

Finasteride mechanism diagram. Reduces DHT production: Blocks 5-alpha reductase, which converts testosterone to DHT. Slows follicle shrinkage: Less DHT reduces the signal that miniaturizes susceptible follicles. Supports hair growth: More hairs enter the growing phase over time.

Oral finasteride for androgenetic scalp hair loss. The main target shown is type II 5-alpha reductase. Finasteride reduces DHT production; it does not block androgen receptors. The larger follicle is a conceptual illustration of growth support, not a guaranteed transformation or creation of new follicles.

How you take it

Treatment plan

The long half-life shapes everything. Levels build over weeks and settle after a few months, so nothing here is judged quickly.

Starting dose: 0.5 mg once a day — the dose used in the hair loss trials and approved for hair where it is licensed.

Alternative schedules: Some prescribers use 0.5 mg two or three times a week, since the drug is still present between doses. Lower exposure, less evidence.

Going up: Nothing above 0.5 mg for this use. Higher doses are not a hair strategy.

Total course: Open-ended. The effect lasts only while you take it, with a long tail after stopping.

The dose does not change with weight, and there is no build-up period. What varies is how often people take it.

Starting dose: 1 mg once a day from day one. Nothing to titrate.

Going up: Nowhere useful to go. Higher doses lower DHT a little further without growing more hair, and the 5 mg prostate dose is not a better hair drug.

Going down: Some doctors use 1 mg every other day, or a few days a week, when side effects are the worry. A reasonable compromise on thinner evidence.

Total course: Open-ended. The effect lasts only as long as you keep taking it.

When to take it

Any time of day, with or without food. Swallow the capsule whole — do not chew, open or squeeze it, and do not use one that is leaking. Wash your hands if you touch a damaged capsule.

Any time of day, with or without food. Attach it to something you already do — two weeks of forgetting is what ruins this drug, not a bad day. Do not split or crush the 1 mg tablet.

If you miss a dose

Take it when you remember, or skip it. Never double up. Because the drug lasts weeks, a missed capsule barely changes your levels — a real advantage over finasteride.

Take it when you remember, or skip it. Never take two at once. One missed tablet makes no difference. A missed month does.

Monitoring

No routine blood tests are needed for a healthy man taking it for hair loss. Monitoring is a review and photographs, as with finasteride.

Two practical points. Dutasteride roughly halves PSA, the prostate blood test, so any doctor ordering one needs to know. Blood donation is deferred for six months after the last capsule, not one month, because it clears so slowly. Nothing accumulates harmfully; the question is whether to continue, not what to test.

No routine blood tests are needed for a healthy man taking 1 mg for hair loss. The honest monitoring is photographs — same spot, same light, every few months, because slow change is invisible day to day.

One number does change. Finasteride roughly halves PSA, the prostate blood test. It does not hide a problem, but the result has to be read knowing you are on it, so tell any doctor who orders one. Nothing needs checking on a schedule; the recurring question is whether you still want to take it.

Months 3 to 6

Months 3 to 6Turning: Shedding slows, and hair feels denser before it looks denser. Sexual side effects, if they happen, show up here.

Months 6 to 12

Months 6 to 12Regrowth: Regrowth appears if it is going to, mostly at the crown and mid-scalp, where dutasteride beat finasteride head to head. The hairline responds too, but stays the hardest area.

Months 6 to 12Regrowth: Regrowth appears if it is going to, mostly at the crown and mid-scalp, fine at first and then thickening. Nearly all measured regrowth happens here.

After 12 months

After 12 monthsJudge it here: Holding what you have is a result. The deeper DHT suppression gives a modest gain over finasteride, not a different category.

After 12 monthsJudge it here: If your hair has simply held, that is a result — holding is what this drug does best. Trials counted more hairs at one year on the drug and fewer on placebo — part of the difference is loss that did not happen.

If you stop

If you stopSlowly backwards: Levels take months to fall, so neither benefit nor side effects disappear quickly.

Cost and getting it

Getting it and cost

Generic dutasteride is cheap, though it costs more than generic finasteride. Branded Avodart costs far more for the same drug.

Hair loss use is off-label in the United States, so insurance rarely covers it and most people pay cash. Compounded topical and injectable versions cost much more on far less evidence.

Generic finasteride 1 mg is inexpensive and widely stocked — one of the cheaper long-term prescriptions in dermatology. Branded Propecia costs considerably more for the same drug.

Telehealth subscriptions bundle the tablet with shipping and a consultation — convenient, and usually more per month than a local generic. Insurance often does not cover it, because hair loss counts as cosmetic, so most people pay cash.

Common questions

Dutasteride:Is it better than finasteride?

At 0.5 mg it regrew somewhat more hair than finasteride at 1 mg. Better is not first choice, though — off-label in the United States, and far slower to clear.

Dutasteride:How long does it stay in my body?

Weeks for most of it, traces for months. That is why donation is deferred six months rather than one.

Dutasteride:Can I switch from finasteride?

That is how most people start it — a planned switch, not an addition.

Dutasteride:Do I need blood tests?

Not routinely if you are healthy. Anyone ordering a PSA needs to know, because the result runs at about half.

Dutasteride:Can my partner handle the capsules?

Intact capsules are fine to handle. Nobody pregnant or who could become pregnant should touch a leaking or damaged one.

Dutasteride:What happens if I stop?

DHT recovers over months, not days, and the hair being held is lost over the following six to twelve months.

Finasteride:How long before I know if it is working?

Six months for the first honest look, twelve for the verdict. Photographs make that possible.

Finasteride:Do I have to take it forever?

As long as you want the effect. Stopping puts you back on your old path within six to twelve months.

Finasteride:Are the sexual side effects permanent?

For almost everyone they do not happen or they resolve. A minority report symptoms that persisted after stopping, and research has neither confirmed nor ruled that out. Worth discussing before you start.

Finasteride:Can my partner touch the tablets?

Whole, coated tablets are fine. Broken or crushed ones should not be touched by anyone pregnant or who could become pregnant.

Finasteride:Is my hair falling out more since I started?

It can, in the first weeks, as weak short hairs are replaced. It usually settles by month three. Still climbing at four months, get it looked at.

Finasteride:Can I take it with minoxidil?

Yes, a common pairing. They work on different parts of the problem, so the effects add up.
