Dermatologist’s verdict
Mixed
An excellent descaler and moisturizer that does not treat psoriasis. Its 2% salicylic acid dissolves the glue holding scale onto the plaque, and thick scale blocks real treatment from reaching the skin underneath. Use it alongside a prescribed treatment, not instead. Psoriasis is an immune disease; nothing here touches that.
What it is
CeraVe Psoriasis Cream is a thick, fragrance-free medicated cream sold over the counter for around $18, for softening and lifting the scale on psoriasis plaques. It is a descaler and a moisturizer, not a treatment for the disease.
The active is salicylic acid 2%, a keratolytic, which dissolves the glue holding dead surface cells together so the scale comes away. The base is the strongest part — three ceramides plus cholesterol and phytosphingosine, with urea, niacinamide, shea butter and hyaluronic acid. It is rich, and it needs to be: a plaque takes a lot of cream.
What it does
Psoriasis is not a skin problem that happens to look inflamed. An overactive immune process makes skin cells multiply far faster than normal, so cells that would take weeks to reach the surface arrive in days. That pile-up is the thick silver scale.
Salicylic acid is a keratolytic. It dissolves the bonds between those dead cells so the scale lifts away. It does nothing to the immune signals underneath, and it has no placebo-controlled evidence as a standalone psoriasis treatment. Its role is to clear scale so an actual drug can get in — a real job, because scale is a barrier.
The rest of the formula is why you would choose it over other descalers. Ceramides, cholesterol and phytosphingosine replace the fats between surface cells, and urea and hyaluronic acid hold water inside them. Moisturizing is genuinely part of psoriasis care, and your plaques will look and feel better and flake less. This is an excellent moisturizer with a descaler in it, used alongside treatment, not instead of it.
What it treats
## Psoriasis | Limited An overactive immune process tells skin cells to multiply far too fast. Cells that should take weeks to reach the surface arrive in days, and that pile-up is the silver scale. Salicylic acid does nothing to the immune process. It dissolves the bonds holding that scale together, and that matters, because thick scale blocks real treatment from reaching the skin underneath. This has the best base in the category, so it descales and moisturizes in one step — alongside your treatment, not instead of it.
## Dry Skin (Xerosis) | Use with care Moisturizing is a genuine part of psoriasis care, and this one is very good, with a descaler in it. Three ceramides, cholesterol and phytosphingosine replace the fats between surface cells, and urea holds water inside them. On plain dry skin it works well, though a cream without salicylic acid stings less on anything cracked.
Best for
**It removes scale, not the disease:** salicylic acid descales psoriasis. It does not treat it.
**The immune process is untouched:** the most important thing on this page. Psoriasis is driven by inflammatory signals that make skin cells multiply far too fast, and salicylic acid does nothing to them. It is a keratolytic — it dissolves the glue holding dead skin together.
**No standalone evidence in psoriasis:** none as a standalone psoriasis treatment. Its legitimate role is as an adjunct — clear the scale so an actual drug can get in.
**It improves how plaques look:** fairly described, an excellent moisturizer with a descaler in it and a monograph claim on the box. Your plaques will look and feel better and flake less. It is not treating your psoriasis.
**Not for large areas of skin:** do not use salicylic acid over large areas. It is absorbed, and high total doses are a genuine concern.
**Know when to see a doctor:** this matters more than which cream you buy.
**Psoriasis affects the whole body:** it is a systemic inflammatory disease, not a skin problem.
**Joint and heart risks are real:** psoriatic arthritis develops in roughly 20–30% of people with psoriasis, and the damage can be permanent if untreated. Psoriasis also raises cardiovascular risk through long-term inflammation.
**Signs that need medical care:** see a doctor if: Any joint pain or stiffness, especially morning stiffness over 30 minutes, or swollen fingers or toes Plaques over about 3–5% of your body (your palm is roughly 1%) It affects your face, scalp, genitals, palms, soles or nails It is not improving after a few weeks It is affecting your sleep or your mental health
**The label warning has a reason:** every one of these products says "ask a doctor if your condition does not improve." That is why.
Texture
Cream A rich, thick cream, heavier than the rest of the CeraVe line, and it needs to be.
It sinks in slowly and leaves a soft film over a plaque, which is the point. On unaffected skin it is too much.
It stings on cracked plaques. Fragrance-free, which matters on inflamed skin.
How to use it
## How to use it How much: Enough to cover the scaly areas, and again after bathing while skin is damp. Careful: Do not spread salicylic acid over large areas. It is absorbed, and at high doses that is a real concern. What to expect: Plaques feel softer within days and flake less within a week or two. The scale returns if you stop; the immune process has not changed. Watch out: See a doctor for joint pain or stiffness, plaques over about 3 to 5% of your body (your palm is roughly 1%), involvement of face, scalp, genitals, palms, soles or nails, or no improvement in a few weeks.

Price per ounce
Softening scale means covering whole plaques twice a day, often on the body. Product goes fast, so size matters more than price per ounce.
The rest of the line
AXIS: Most exfoliating | Purely moisturizing
CeraVe makes two psoriasis products and two more using the same acid. All remove scale. None treat the disease.
**Psoriasis Moisturizing Cream (this one)** — Salicylic acid 2%, with urea and lactic acid. The most exfoliating of the four.
**Psoriasis Cleanser | cerave-psoriasis-cleanser** — The same 2% salicylic acid in a wash. It rinses off, so it does less.
**SA Cream for Rough & Bumpy Skin | cerave-sa-cream-for-rough-and-bumpy-skin** — Salicylic and lactic acid, no Drug Facts panel. A cosmetic, not a drug.
**Moisturizing Cream | cerave-moisturizing-cream** — No exfoliant. For between flares, or on raw skin.

These are the options if you want a stronger descaler, a coal tar product, or a plain rich moisturizer. None treats psoriasis either.
MG217 Psoriasis Ointment | more | Coal tar 2% in a petrolatum ointment, and coal tar has real evidence behind it. Messy, it stains, and it raises sun sensitivity. Psoriasin Deep Moisturizing Ointment | more | Coal tar 2% in a petrolatum base, a real psoriasis drug. Adults only, and it raises sun sensitivity. Gold Bond Psoriasis Relief | same | Salicylic acid 3%, the highest the monograph allows, in a decent ceramide cream. Same limits — scale, not disease.
