Thermage

Thermage heats the deeper layers with radiofrequency. The tip is stamped across the area.
Thermage is the one with no needles and no recovery. It heats a block of skin from the outside, in a single session, and it is aimed at crepey texture and mild slackness. Morpheus8 pushes fine needles through the skin to place heat underneath. It is a course of about three sessions with a few days of redness each time, and it treats acne scars and rough texture as well as mild laxity. Pick Thermage if you cannot take downtime, and Morpheus8 if the surface of your skin also needs work.

Thermage heats the deeper layers with radiofrequency. The tip is stamped across the area.

Morpheus8 uses a grid of fine needles that carry heat below the surface. Depth is set for each area.
Side by side
Thermage
Radiofrequency heat through the skin
Current passes through the tissue to a pad on your back. The skin's own resistance to that current makes the heat.
Morpheus8
Needles that carry heat under the skin
An array of fine pins enters the skin and delivers radiofrequency at the tip, so the heat is placed rather than aimed.
Thermage
A block of tissue, not a plane
Warmest in the skin, spreading down. It cannot be set to a chosen depth.
Morpheus8
Set by how far the needles go
Commonly 1 to 4 mm on the face, deeper with the larger tips used on the body.
Thermage
Firmer, better-quality skin
Crepey texture, fine wrinkles and mild slackness, including thin skin around the eyes.
Morpheus8
Smoother skin and a firmer edge
Acne scars, rough texture and large pores, plus mild tightening along the jaw.
Thermage
It does nothing for acne scars
The surface is never broken, so scarring and texture are untouched.
Morpheus8
It is not an eyelid treatment
The needles are not used on the lid itself.
Thermage
A hot flick, over and over
The tip is stamped across the area, with a spray of coolant before and after each pulse. Uncomfortable rather than dreaded.
Morpheus8
Pressure, heat and pinpricks
Numbing cream first. The skin feels hot and sore for a few hours afterwards.
Thermage
Usually one
Repeated after a year or two if you liked it.
Morpheus8
Usually three, a month apart
Some plans run to four or five. Ask for the number before you start.
Thermage
Two to six months
Collagen contracts a little on the day, then rebuilds slowly.
Morpheus8
About three months after the last visit
Each session adds a little, and the build continues afterwards.
Thermage
About one to two years

About one to two years
Thermage
None
Redness and slight swelling for a few hours. Make-up the same day is normal.
Morpheus8
A few days
Pink skin with a faint grid of pinpoint dots. Make-up usually covers it by day three.
Thermage
Priced per treatment tip and area
Morpheus8
Priced per session, and it is a course
Thermage
Home radiofrequency wands are far weaker
They warm the top of the skin a little. The clinic device drives current through the full thickness with a return pad, which is a different scale of heating.
Morpheus8
A home roller is a different thing
It pricks the surface with much shorter needles and adds no heat at all. Heat is the whole point of this treatment.
Thermage
Thermage never breaks the skin. That is the practical reason people choose it. You are treated and you go back to work with nothing on your face to explain.
Morpheus8
Morpheus8 breaks the skin on purpose. The needles are how the heat reaches the right depth, and the healing they trigger is part of the result. The trade is a few days of redness and small dots.
Thermage
Thermage heats a volume and cannot pick a layer. Current spreads through the tissue, hottest near the surface, with a coolant spray protecting the top. That is fine when the whole area needs firming.
Morpheus8
Morpheus8 sets its depth by needle length. An operator can treat shallow for texture or deeper for firmness, and change it across the face in one session.
Thermage
Thermage has been studied for over twenty years. The result is modest and gradual, and the device is well described.
Thermage
Thermage's rare problems come from heat concentrating too much. Burns and blistering are uncommon. So is fat loss under the skin, which leaves a small dent.
Morpheus8
Morpheus8's rare problems come from depth and energy. Small tracks or texture changes where needles went in, dark marks that fade over months, and loss of fat under the skin if the settings were too aggressive for a thin face.
Common questions
They answer different complaints. Thermage suits crepey, thin skin when you cannot take any downtime. Morpheus8 suits acne scars and rough texture, and needs a few days of redness after each session.
Neither is strong here. Morpheus8 can be set deeper and may firm a soft jawline a little more, but skin that hangs in a fold is a surgical problem.
The sensations differ more than the intensity. Morpheus8 is pressure and pinpricks under numbing cream, then hours of soreness. Thermage is a repeated flick of heat during the session and nothing much afterwards.
Thermage is usually one, repeated after a year or two. Morpheus8 is usually three, about a month apart, so compare the total cost rather than the price per visit.
Some clinics do, spaced apart, because one works on the surface and the other on overall firmness. There is no published evidence that combining them beats doing one well.
Neither uses light, so neither targets pigment, and both are reasonable across skin tones. With Morpheus8, ask what energy setting is planned, since heat in the skin can still leave dark marks while it heals.
RF microneedling can cause serious injury. The FDA has received reports of burns, scarring, loss of fat, nerve damage and disfigurement. Some injuries have needed surgery or other medical treatment. A few days of expected redness is not the full risk picture.
Choose a licensed medical professional trained on the device. Discuss the risks and other treatment options before deciding. RF microneedling should not be done at home.
