Ultherapy

Ultherapy uses focused ultrasound. The handpiece is moved along the jawline and neck in lines.
Ultherapy is not a fat treatment. It uses focused ultrasound to heat deep layers of skin, so the skin tightens and lifts over the following months. CoolSculpting is only a fat treatment. It freezes a pinched fold of fat and leaves skin exactly as it found it. These two solve different problems, and the only place people genuinely have to choose is under the chin.

| What it treats | Skin that has started to droop | Fat you can pinch |
| What it feels like | Sharp prickles of heat, in bursts | A hard pull, then deep cold |

Ultherapy uses focused ultrasound. The handpiece is moved along the jawline and neck in lines.

The applicator sits on the treatment area and cools the fat beneath it. The skin surface is protected.
Side by side
Ultherapy
Skin that has started to droop
Brow, jawline, under the chin, the neck, and lines on the chest.
CoolSculpting
Fat you can pinch
Stomach, flanks, thighs, upper arms, under the chin.
Ultherapy
Heat, delivered under the skin
Ultrasound is focused at set depths. The heat prompts your body to lay down collagen, and the tissue tightens as it does.
CoolSculpting
A cup suctions and freezes fat
The applicator holds a fold of fat cold for about 35 to 45 minutes. Your body clears the damaged cells over weeks.
Ultherapy
Sharp prickles of heat, in bursts
The jawline and forehead are the uncomfortable spots. It is short, but people do not describe it as relaxing.
CoolSculpting
A hard pull, then deep cold
The cold fades to numbness within about ten minutes. The massage at the end is the part people remember.
Ultherapy
Usually one
Then a repeat after a year or two if you want to hold it.
CoolSculpting
Usually two rounds per area
Spaced four to eight weeks apart.
Ultherapy
Two to three months
Full effect by around six months. It is a slow build.
CoolSculpting
Six to twelve weeks
Ultherapy
About one to two years
Your skin keeps ageing underneath, so the lift is borrowed time.
CoolSculpting
The treated fat cells are gone
Destroyed fat cells do not return, though the ones left can enlarge if you gain weight.
Ultherapy
Uncomfortable, and results vary
Some people see a clear lift, some see very little. There is no way to know beforehand.
CoolSculpting
Fat can rarely grow instead
Paradoxical adipose hyperplasia: months later the treated area becomes firmer and larger rather than smaller. It affects about 0.22% of people, roughly 1 in 455, and liposuction corrects it.
Ultherapy
Priced by area, usually one payment
CoolSculpting
Priced per cycle, and plans use several
Ultherapy
Home wands go nowhere near as deep
Ultherapy focuses heat at set depths under the skin. A handheld device puts a fraction of that energy into the surface and cannot reach the layer that does the lifting.
CoolSculpting
Freezer wraps are a different thing
A gel pack starts cold and warms up as you wear it. The clinic device holds one set temperature for the whole session, and that steady cold is what damages the fat cell.

Ultherapy
If the problem is slack skin and a soft jawline, Ultherapy is the direct answer. Freezing fat there will not sharpen an outline that has gone soft.

Focused ultrasound heats a deep layer to tighten it, aimed at the jaw, neck and brow rather than at lines. The change is modest and takes three to six months to show, and it is nothing like a surgical lift. It can be uncomfortable, and results vary enough that it is worth seeing a provider's own before-and-after photographs.
Common questions
Fat freezing can occasionally make the treated fat bulge grow rather than shrink, called paradoxical adipose hyperplasia. Ask about this risk and the options if it happens. Other technologies have their own risks; compare the exact devices and intended treatment areas.
