CoolSculpting vs Liposuction

CoolSculpting is better when you want a small change and no time off. It freezes a pinched fold of fat through the skin, and most people go back to work the same day. Liposuction is better when you want a clear change in one go. A surgeon suctions the fat out through tiny cuts, so far more comes off in a single session, but it is an operation with a real recovery. Neither one tightens loose skin.

10 comparison points

What actually happens

CoolSculpting

A cup suctions and freezes fat

The applicator holds a fold of tissue cold for about 35 to 45 minutes. Your body clears the damaged fat cells over the following weeks.

Liposuction

A thin tube suctions the fat out

The surgeon numbs the area with fluid, makes small cuts, and moves a cannula through the fat layer to draw it out.

How much it changes

CoolSculpting

A modest change in one area

Enough to see in clothes and in photos. Not enough to change a dress size.

Liposuction

A large change, decided in the room

The surgeon can take a lot or a little and can shape as they go. This is the flatter result most people picture.

Anaesthetic

CoolSculpting

None

Nothing is injected and nothing is numbed. The cold does the numbing on its own.

Liposuction

Local, sedation or general

Small areas can be done awake. Bigger plans usually mean sedation or a general anaesthetic.

Recovery

CoolSculpting

None to speak of

The area is numb, red and firm for days to weeks. A deep ache can arrive a few days later.

Liposuction

Days off, then weeks of swelling

Most people take about a week away from normal life, wear a compression garment for several weeks, and wait three to six months for the shape to settle.

How long until you see it

CoolSculpting

Six to twelve weeks

Usually two rounds per area, spaced four to eight weeks apart.

Liposuction

Weeks to months

Bruising and swelling hide the result at first. The final shape is judged at three to six months.

The main downside

CoolSculpting

Fat can rarely grow instead

Paradoxical adipose hyperplasia: months later the treated area becomes firmer and larger rather than smaller. It affects about 0.22% of people, roughly 1 in 455, and liposuction corrects it.

Liposuction

Dents, ripples and uneven edges

Taking too much from one spot, or not blending into the next, leaves a contour you can see. Fixing it is harder than getting it right the first time.

The at-home or drugstore version

CoolSculpting

Freezer wraps are a different thing

A gel pack starts cold and warms up while you wear it. The clinic device holds one set temperature for the whole session, and that steady cold is what damages the fat cell.

Liposuction

There is no home version

Nothing you can buy removes fat from under the skin. Massage devices, wraps and creams move fluid for a few hours.

Who each suits

CoolSculpting

  • Best for — A pinchable roll on the belly, flanks, arms or under the chin, with firm skin over it.
  • The mindset — You want one area smaller, and you do not want anyone to know.

Liposuction

  • Best for — Several areas at once, a larger volume, or a shape that needs blending.
  • The mindset — You accept an operation and a recovery for a bigger, faster change.

Common questions

Questions Patients Ask

Is CoolSculpting as good as liposuction?

No, and it is not meant to be. Liposuction removes far more fat in one session and lets the surgeon shape the area as they go. CoolSculpting thins one fold, a bit at a time.

How much fat does CoolSculpting remove compared to liposuction?

CoolSculpting thins the fat layer under the applicator by a modest amount, and most areas need more than one round. Liposuction removes a much larger volume in a single operation.

Does the fat come back after either one?

The fat cells that are destroyed or removed do not come back with either treatment. The ones left behind can still enlarge, so a weight gain will show.

Which one hurts more?

Liposuction, but you are numbed or asleep for it. CoolSculpting is uncomfortable for the first ten minutes, then the area goes numb, and a deep ache can turn up a few days later.

Can I have CoolSculpting instead of liposuction if I have loose skin?

Neither one helps loose skin. Taking fat out from under skin that already hangs can make it look slightly looser, so ask about your skin before you choose either.

Which costs more?

Liposuction costs several times more than a round of CoolSculpting. A full CoolSculpting plan uses several cycles, so compare the whole course price rather than the price of one cycle.
