truSculpt iD

truSculpt iD is better if the only thing you want is less fat over the stomach. It is one or two 15-minute sessions of heat, with no muscle contraction. EmSculpt NEO is better if you want the muscle worked as well. It is four 30-minute sessions that force the muscle to contract and heat the tissue at the same time. Neither one is a weight-loss treatment.
| The difference | truSculpt iD | EmSculpt NEO |

| Cost | Fewer, shorter appointments | Sold as a course of four |

Four 30-minute sessions, 5 to 10 days apart A paddle is strapped over the area. A pulsed magnetic field makes the muscle contract, and radiofrequency supplies heat at the same moment. BTL does not publish instructions for use, so there is no labelled session count. Its website says four.
Side by side
truSculpt iD
One or two 15-minute sessions
A handpiece heats the fat layer from outside the skin. There is no muscle stimulation in it. The session count comes from the maker and the clinic, not from a label.
EmSculpt NEO
Four 30-minute sessions, 5 to 10 days apart
A paddle is strapped over the area. A pulsed magnetic field makes the muscle contract, and radiofrequency supplies heat at the same moment. BTL does not publish instructions for use, so there is no labelled session count. Its website says four.
truSculpt iD
Less fat over the abdomen
The tissue is heated enough to damage fat cells, which the body then clears. Expect a subtle change in the fat layer, not a new shape.
EmSculpt NEO
Less fat, and some muscle built
A 2025 review found mean fat thickness fell 8.0 mm and mean muscle thickness rose 5.5 mm. It is the only one of the two that does anything to muscle.
truSculpt iD
No muscle effect at all
It is a single-purpose heat device. Cutera's muscle device is a different machine, truSculpt flex, which uses sticky electrodes.
EmSculpt NEO
No fat claim for buttocks or calves
Its muscle wording covers those two areas. Its fat wording does not.
truSculpt iD
8 to 12 weeks
People in the published study were measured 8 and 12 weeks after a single session.
EmSculpt NEO
Two to three months after the last session
The course comes first, then the waiting.
truSculpt iD
No published schedule
Cutera publishes no maintenance protocol. The intervals clinics quote are convention.
EmSculpt NEO
No published schedule
BTL publishes no maintenance schedule either.
truSculpt iD
Building heat, no contraction
Clinics describe it as a hot stone massage. Expect warmth and redness afterwards. The published study reported no downtime.
EmSculpt NEO
Forced contractions, then soreness
Intensity is ramped up to the highest level you can bear. In the abdominal study, 11 of 42 people reported muscle soreness afterwards. 93% reported no more than minimal discomfort during the treatment itself.
truSculpt iD
Fewer, shorter appointments
One or two 15-minute visits rather than a course of four.
EmSculpt NEO
Sold as a course of four
Ask what happens if you buy a package and stop partway.
truSculpt iD
No home version of this
Home radiofrequency wands put a fraction of the power into the surface of the skin. They do not reach the fat layer.
EmSculpt NEO
Home ab stimulators are a different thing
Stick-on ab belts run a mild current through pads on the skin. That reaches the muscle just under the surface, and adds no heat. It is a different treatment with a similar name, not a weaker version of this one.
truSculpt iD
EmSculpt NEO
EmSculpt NEO
EmSculpt NEO covers more of the body, and covers it in two different ways. Its fat wording runs to four areas. Its muscle wording runs to five. The two lists are not the same list.
truSculpt iD
truSculpt iD has less research behind it. There are two small published studies. Neither used a sham comparison.
EmSculpt NEO
Almost all the EmSculpt NEO research was written by the maker's own advisors. A 2025 review found seven NEO studies. All seven were written by BTL medical advisors.
truSculpt iD
truSculpt iD's fat wording covers the abdomen only. The cleared indication gives the 2 MHz setting on the 40 cm2 handpiece "reduction in circumference of the abdomen and non-invasive lipolysis (breakdown of fat) of the abdomen" (Cutera, K221407, 13 July 2022). There is no thigh, flank, arm or buttock fat wording in it, and no muscle claim of any kind. The rest of that clearance is medical heating wording, for pain, muscle spasm and local circulation.
EmSculpt NEO
EmSculpt NEO's wording is split across two different regulations. The fat and circumference wording sits on radiofrequency filings: abdomen in December 2019, thighs in January 2021, upper arms in January 2022, and flanks in September 2023. The muscle wording covers abdomen, buttocks, thighs, calves and arms, and sits on separate electromagnetic filings that carry no fat claim at all. There is no fat clearance for buttocks or calves.
Common questions
Neither is a weight-loss treatment. FDA says this about the whole category of non-invasive body contouring.
Tell your provider about any implant. That means a pacemaker, an implanted defibrillator, a neurostimulator, a drug pump, or metal near the area to be treated. Both devices exclude implants. Radiofrequency heats metal, and magnetic stimulation induces a current in it. Mention a hernia in or near the area too, including a repaired one. FDA names that as a serious risk for this class of device.
Ask which body area the treatment is cleared for. For truSculpt iD, the fat wording applies to the abdomen only. For EmSculpt NEO, ask whether you are being treated for fat, for muscle, or both, and where.
Both devices are FDA cleared, not FDA approved. A 510(k) clearance is a finding that a device is much the same as one already on the market. It is not a finding that it works well.
BTL does not file with FDA under its trade names. It files under model numbers such as BTL-899 and BTL-703-2, so no clearance number is attached to the name "EmSculpt NEO."
Marketed areas are not always cleared areas. BTL's website markets "back" and "legs" as treatment zones. Those are not in the cleared list. A clinician may legally treat outside a clearance, but FDA reviewed no evidence for those uses.
