Sculptra vs Botox

Sculptra is the better choice for a face that looks deflated. It is a course of injections that prompts your own body to build collagen over months. Botox Cosmetic is the better choice for lines made by muscle movement. It is one appointment per area, it works within days, and it wears off. No trial has compared the two, and neither one does the other's job.

Sculptra

A gloved injector places a needle into the cheek of a woman lying back, with small blue dots marked on her skin.

Sculptra is injected at marked points across the cheek. It adds very little volume on the day — the change comes from collagen built over the months after.

Botox

Gloved hands steady a woman's face while a fine needle on a small syringe is placed into her cheek.

An injectable treatment being placed in the face. A neurotoxin is given with a very fine needle in small amounts, and each spot takes only a few seconds.

The differences that matter

12 comparison points

What you actually do

Sculptra

Up to four sessions, weeks apart

A powder mixed with sterile water, then injected into the deep dermis. In the trials behind its approval, people had up to four sessions, spaced three to four weeks apart. Most needed more than one.

Botox

One appointment per area

A prescription botulinum toxin injected into named muscles, repeated when it wears off. How many units you get depends on how many areas are treated.

What you can expect to see

Sculptra

Fuller skin, built from your own collagen

The water it is mixed with is absorbed within days. What is left behind is a scaffold of tiny particles. Your body responds by laying down new collagen around them, so the volume you end up with is your own tissue.

Botox

Lines that stop folding

It blocks the nerve signal that tells the muscle to contract. The injected muscle stops squeezing fully, so the skin above it stops creasing.

What it will not do

Sculptra

Not for frown lines

It builds collagen. It does not switch off a muscle. It is also placed in the deep dermis, which is not where a movement line is treated.

Botox

Not for lost volume

It takes up no space and builds no collagen. Relaxing a muscle in a deflated area does not put anything back.

How long before you see anything

Sculptra

Weeks to months, across a course

Almost nothing shows on the day. The label even warns injectors not to overfill, because the dent is expected to improve gradually over the weeks after injection.

Botox

One to two days, judged at a month

The label says the muscle starts to weaken one to two days after injection, and the effect grows through the first week. Trials judged the result at day 30.

How long it lasts

Sculptra

Followed out to about two years

The nasolabial fold trial followed people to month 25. The cheek trial followed them to month 24. Those are where the studies stopped.

Botox

About three to four months

The label puts the duration for frown lines at about 3 to 4 months. In the trials, 80% were rated none or mild at day 30 and 25% at day 120.

The downside

Sculptra

Delayed lumps, and nothing dissolves it

Firm lumps can appear months after treatment. There is no reversal agent.

Botox

It wears off, so you repeat it

Nothing reverses it either, but nothing is left behind in the tissue.

Cost

Sculptra

Paid per session, and it is a course

Price a course of three or four sessions, not one visit.

Botox

Paid per area, repeated over time

An ongoing cost rather than a one-off one.

Who tends to be happy with it

Sculptra

  • Best for — Someone whose face reads as deflated rather than lined, who will pay for a course and wait months to judge it.
  • The mindset — A slow rebuild, not a before-and-after on the day.

Botox

  • Best for — Someone whose lines vanish when they hold their face still.
  • The mindset — A short, repeatable maintenance treatment aimed at movement.

The two clocks

Sculptra

Sculptra shows almost nothing at first. The water it is mixed with is absorbed within days, so any early fullness goes with it. A course runs to as many as four sessions, three to four weeks apart, so treatment alone takes two to three months. People who are unhappy with Sculptra very often judged it too early.

Botox

Botox Cosmetic then fades. Its label puts the duration for frown lines at about 3 to 4 months. In the trials, 80% of treated people had no more than mild lines at day 30, 70% at day 60, 48% at day 90 and 25% at day 120.

The lumps, and why they matter more here

Sculptra

Sculptra's signature problem is a lump that shows up months later. In the nasolabial fold trial, 17% of the Sculptra group had a nodule or a papule, against 13% of the group given a collagen filler. Most could be felt rather than seen, and caused no symptoms.

Sculptra cannot be dissolved. Hyaluronidase breaks down a hyaluronic acid filler. It has nothing to act on in poly-L-lactic acid. The particles do break down on their own over time, so Sculptra is not permanent — but not permanent and not reversible are different things.

What each costs

Sculptra

Those numbers understate Sculptra. It is sold as a course, so a single-session figure is not what you would pay.

The fine print

Sculptra

Sculptra is a Class III device made by Galderma, approved through the FDA's premarket approval route as PMA P030050. The 2004 approval covers facial fat loss in people with HIV. The 2009 approval covers nasolabial folds and other facial wrinkles treated with a deep dermal cross-hatch technique, in people with normal immune function. An April 2023 approval added fine lines and wrinkles in the cheek.

The 2009 approval was for a course, not a single session: up to four sessions at about three-week intervals, with a maximum of 2.5 mL per nasolabial fold per session. The 2023 cheek study used four-week intervals.

Sculptra has no approved reversal agent.

Botox

Botox Cosmetic is made by Allergan Aesthetics, an AbbVie company. It is approved for four cosmetic areas: glabellar lines in 2002, lateral canthal lines in 2013, forehead lines in 2017 and platysma bands in 2024. Botox Cosmetic Units do not convert into Dysport or Xeomin units.

Common questions

Questions Patients Ask

How long until Sculptra actually shows?

Not on the day. The water it is mixed with is absorbed within days and any immediate fullness goes with it. The approved regimen runs to four sessions about three weeks apart, so the full result is not in place before roughly three to six months from the first session. Judging it at four weeks is judging it too early.

How long does each one last?

Sculptra's nasolabial fold trial followed people to month 25 and its cheek trial to month 24, and those are where the studies stopped rather than where the effect ended. The Botox Cosmetic label puts no number on duration and simply calls the improvement temporary; published trials measured it at day 30 and day 60, and one three-brand comparison followed people for 180 days.

Can Sculptra replace Botox?

No. Sculptra builds collagen and adds volume over months. It does nothing to a muscle, so a frown line that appears only when you frown will still appear. Their FDA-approved areas do not overlap at any point on the face.

Can Botox replace Sculptra?

No. Botulinum toxin occupies no space and adds no volume, so it cannot fill a hollow temple, a flat cheek or a deep fold caused by lost tissue.

Can Sculptra be dissolved if I do not like it?

No. Hyaluronidase breaks down hyaluronic acid and has no target in poly-L-lactic acid, and the Sculptra instructions for use name no reversal agent. Documented options for an unwanted result are time, saline infiltration with mechanical disruption, corticosteroids and, rarely, surgery.

Which one is riskier?

They fail in different ways. Sculptra's characteristic problem is a delayed lump — reported in 17.2% of the Sculptra arm in the 2009 nasolabial fold trial and in 1 of 97 treated subjects analysed in the 2023 cheek trial, counted differently in each. Both carry the rare emergency risk of an injection entering a blood vessel. Botox Cosmetic leaves nothing behind and wears off.

Has anyone compared them directly?

No. There is no head-to-head trial of Sculptra against Botox Cosmetic, and there is no head-to-head trial of any filler against any botulinum toxin. They treat different problems, so numbers from their separate trials cannot be lined up to pick a winner.

Can I have both?

Clinics commonly offer both, and they are aimed at different parts of the face. Ask for them to be planned as separate treatments with their own timelines, since one is judged at two weeks and the other at three months.

Safety to keep in mind

The timing is the part people are rarely told. In that trial, nodules appeared a median of 160 days after the first session, more than five months. They then lasted a median of 100 days.

The rate depends a lot on where it is injected. FDA required a follow-up study after approval. Of 867 treated people reported through year 3, 28% had a nodule or papule. By area it was 27% in the marionette lines, 19% in the nasolabial folds, 5% in the cheek folds and 4% in the chin crease.
