Radiesse vs Botox

Botox Cosmetic relaxes the muscles that make lines when you move. It is used for frown lines, lines at the outer corner of the eye, forehead lines and neck cords, and it wears off over months. Radiesse is a calcium-based filler that adds volume where the face has lost it. It builds out a jawline or the back of the hands and prompts your body to make collagen — and it cannot be dissolved.

Radiesse

A practitioner injects the cheek of a woman whose face is marked with a white grid, in a clinic treatment room.

Radiesse being injected along mapped-out lines. The white grid drawn on the face shows where the product will go before any needle enters.

Botox

Gloved hands steady a woman's face while a fine needle on a small syringe is placed into her cheek.

An injectable treatment being placed in the face. A neurotoxin is given with a very fine needle in small amounts, and each spot takes only a few seconds.

The differences that matter

9 comparison points

What you actually do

Radiesse

An injection that adds volume

Radiesse is calcium hydroxylapatite, a mineral ground into tiny spheres and mixed into a gel. It is injected deep, under the skin or onto the bone beneath it.

Botox

An injection that relaxes muscle

Botox Cosmetic is a prescription botulinum toxin. It is given as several small shots into the muscles that make the line. How many shots you need depends on how many areas are treated.

What you can expect to see

Radiesse

Volume straight away, then collagen

The gel gives volume right away. Your body absorbs the gel over the first two to three months. The mineral spheres stay behind, and your body builds collagen around them before breaking them down.

Botox

Lines that stop forming

The muscle stops contracting for a while, so the skin over it stops folding. Nothing is added underneath.

What it will not do

Radiesse

It will not lift lax skin

Filler does not lift skin that has become lax, so it does not treat jowls. The label also says it has not been shown to be safe or effective in the lips, and nodules have been reported there.

Botox

It adds no volume

It relaxes muscle and nothing else. In the neck it treats the cords that stand out when you tense, not the skin quality around them.

If you want it undone

Radiesse

It cannot be dissolved

Hyaluronidase is the enzyme that dissolves hyaluronic acid fillers. Radiesse has none in it, so the enzyme has nothing to act on. The options are time, saline broken up through a cannula, steroid shots and, rarely, surgery.

Botox

No antidote, but it wears off

There is no reversal agent for botulinum toxin either. It fades on its own over months and takes nothing with it.

How long it lasts

Radiesse

Months to a few years, measured

In one trial, 40% of treated folds were still graded improved at 30 months or more after the last treatment. That means 60% were not. The FDA's consumer page puts calcium hydroxylapatite at about 18 months.

Botox

The label puts no number on it

The label calls the improvement temporary. It gives no duration figure at all.

The at-home or drugstore version

Radiesse

There is no home filler

Nothing sold over the counter puts a gel under your skin. Lip plumpers mildly irritate the lip, so it swells for a few hours. That is not the same thing.

Botox

There is no Botox in a bottle

The peptide serums sold under that name sit on the surface. The muscle is several millimetres down, and a cream does not get there.

Who tends to be happy with it

Radiesse

  • Best for — Someone who wants structural support along the jawline or in the back of the hands.
  • The mindset — A firmer result that lasts longer. But it is a one-way door.

Botox

  • Best for — Someone whose complaint is a line that only appears with movement, or neck cords that stand out when they tense.
  • The mindset — Reversible by default, because it fades.

Why "reversible" does not apply here

Botox

Botox Cosmetic has no antidote either. The difference is that it fades in months and takes nothing with it.

The fine print

Radiesse

Radiesse is FDA approved as a Class III device. The 2006 approval covers "subdermal implantation for the correction of moderate to severe facial wrinkles and folds, such as nasolabial folds". Hand augmentation was added in 2015, jawline contour for Radiesse (+) Lidocaine in 2021, and wrinkles on the chest at a 1:2 dilution with saline in 2026. It is also approved for facial fat loss in HIV. It is not approved for jowls, marionette lines or the temples by name.

Radiesse composition. Calcium hydroxylapatite spheres 25 to 45 microns across, in a gel of sterile water, glycerin and carboxymethylcellulose.

The Radiesse label sets a limit on how long it has been studied. It states only that safety beyond 3 years in the face and 2 years in the hand has not been investigated.

Botox

Botox Cosmetic is onabotulinumtoxinA, a botulinum toxin type A. It is approved for four areas: frown lines between the brows in 2002, lines at the outer corner of the eye in 2013, forehead lines in 2017, and the neck cords called platysma bands on 18 October 2024.

Label doses are given in Botox Cosmetic Units only. They are 20 Units, 24 Units, 40 Units in total for the forehead together with the frown lines, and 26, 31 or 36 Units for the neck cords depending on severity. These units do not convert to any other brand.

Common questions

Questions Patients Ask

Can Radiesse be dissolved?

No. Hyaluronidase only breaks down hyaluronic acid, and Radiesse contains none — it is calcium hydroxylapatite microspheres in a carboxymethylcellulose gel. There is no approved reversal agent, and the current labelling names none.

What about sodium thiosulfate?

It is not established. A 12-sample cadaveric porcine study and some case reports support it, but the largest preclinical study, run by the manufacturer, found no degradation of calcium hydroxylapatite in the laboratory or in live animals and did observe necrosis and haemorrhage. A separate cadaveric study found CaHA still present in facial artery segments after 24 hours regardless of concentration.

Is Radiesse permanent, then?

No — and that is a different question from whether it is reversible. The microspheres break down into calcium and phosphate that the body handles normally. What you cannot do is make that happen on demand. The label says only that safety beyond 3 years in the face and 2 years in the hand has not been investigated.

Which one is better for the jawline?

They do different things. Radiesse (+) Lidocaine has been approved since 2021 to improve moderate to severe loss of jawline contour, and 75.6% of treated people responded at week 12 against 8.8% of controls. Botox Cosmetic has no jawline indication; its neck approval is for platysma bands. Neither treats jowls, and no filler lifts lax skin.

Can Radiesse be used in marionette lines or the temples?

Marionette lines are not named in any Radiesse approval, though the general wording "facial wrinkles and folds, such as nasolabial folds" is an example rather than a limit and plausibly covers them — with no trial evidence specific to that area. Radiesse is not approved for the temples. Off-label use is legal and routine, but it means the trial figures do not apply there.

Which lasts longer?

There is no answer from published evidence, because no trial has compared them and they are not measuring the same thing. Radiesse's longest measurement found 40% of nasolabial folds still graded "improved" or better at 30 months or more after the last treatment - so 60% were not. The FDA's consumer page puts calcium hydroxylapatite at approximately 18 months. The Botox Cosmetic label gives no duration figure at all and calls the improvement temporary.

Is Radiesse riskier than a dissolvable filler?

The rare emergency — filler entering a blood vessel — applies to both, but it cannot be treated with hyaluronidase when the product is calcium hydroxylapatite. That is a real difference in what can be done in an emergency, and it is worth weighing before choosing a product that is not dissolvable.

Should I have Radiesse in my lips?

The Radiesse labelling states that safety and effectiveness for use in the lips has not been established, and notes published reports of nodules after injection into the lips. Consensus reviews advise avoiding calcium hydroxylapatite in highly mobile areas such as the lips and in the area around the eye.
