EZ Gel vs Sculptra

Ez Gel is made from your own blood, in the room. Blood is drawn and spun, and part of it is heated into an injectable gel. Sculptra is a synthetic polymer made in a factory and approved by the FDA. It is poly-L-lactic acid, supplied as a freeze-dried powder and mixed with sterile water before it is injected.

What you actually do

Ez Gel

Blood drawn and spun, then injected

Your blood is drawn and spun in a centrifuge in the treatment room. Heating the albumin layer turns it into a gel. The gel is then mixed with liquid platelet-rich fibrin and injected.

Sculptra

A course of three or four sessions

A vial of freeze-dried powder is mixed with sterile water. It is injected into the deep dermis using a threading technique. The result is judged at three months.

How predictable each dose is

Ez Gel

It varies with the person and the clinic

There is no product specification, no lot number and no label, because there is no product — only a process. Two clinics using the same name may not be producing the same gel.

Sculptra

The same every time

The dilution, the depth and the maximum volume per area are all written down in documents anyone can read.

What you can expect it to do

Ez Gel

Soft, conservative correction

The technique paper aims it at people who want a conservative result, most often around the eyes.

Sculptra

Facial volume and wrinkles

Sculptra is injected into the deep dermis, in the named approved areas. The under-eye area is not one of them.

How long it lasts

Ez Gel

Measured in animals, not in people

In a mouse study, ordinary platelet-rich fibrin was largely gone by 21 days, while the albumin version held its volume. There is no equivalent figure in people.

Sculptra

Measured in people, past two years

Wrinkle scores stayed stable or improved out to month 25 in a trial extension.

What is left behind afterwards

Ez Gel

Nothing synthetic

The material is your own protein and the body resorbs it. That is the genuine advantage of this side.

Sculptra

Your own collagen

The particles break down over roughly two years. The collagen built around them stays, and it cannot be removed.

The downside

Ez Gel

No label and no agreed maximum dose

There is no reviewed label, no agreed maximum volume and no defined depth. The quality of the treatment is the injector's.

Sculptra

Lumps that can appear months later

Papules or nodules occurred in 17% of the Sculptra group in the 2009 nasolabial fold trial. The labelling records nodules starting at a median of 160 days. It cannot be undone.

The at-home or drugstore version

Ez Gel

There is no home version

The gel only exists after blood is drawn and spun. There is nothing to buy over the counter.

Sculptra

There is no home version

Sculptra places a polymer several millimetres down in the skin. A cream cannot reach that depth.

Who tends to be happy with it

Ez Gel

  • Best for — Someone who does not want a synthetic material in their face at all, and who is treating a small soft area.
  • The mindset — A conservative treatment with no controlled trial behind it.

Sculptra

  • Best for — Someone whose face reads as deflated, who wants a documented product with reviewed trials.
  • The mindset — A course of three or four sessions, judged at three months.

What Sculptra gives up in exchange for being manufactured

Sculptra

Sculptra works because your body reacts to it. The treatment is a controlled response to a foreign material. That is the mechanism, not a side effect.

No one has compared them

Sculptra

Sculptra's numbers come from randomized trials. In a cheek trial of 149 people, 71% of the treated group were rated responders at month 12, against 26% of the untreated group. The maker paid for the trials.

The fine print

Ez Gel

There is no FDA-approved or cleared injectable filler product called Ez Gel. The material is made from the patient's own blood during the visit, so there is no product to approve. The centrifuge or kit used may hold its own clearance for preparing blood components, but a clearance for a preparation device is not approval of a cosmetic treatment. No clearance covers an Ez Gel kit for aesthetic injection, so a clearance number quoted for it will be for something else.

The published Ez Gel protocol spins the albumin at 2,600 rpm for 7 minutes and the fibrin at 1,100 rpm for 5 minutes, then mixes them at roughly 3.5 to 1.5 cc. The family of techniques goes by several names, including albumin gel, Bio-Heat and Alb-PRF.

Sculptra

Sculptra is supplied by Galderma as a freeze-dried powder. Each vial holds 367.5 mg of poly-L-lactic acid microparticles with sodium carboxymethylcellulose and mannitol, reconstituted with 5 mL or 8 mL of sterile water.

Sculptra is a Class III device approved through premarket approval, PMA P030050. On that route the FDA reviews the product and the trials behind it. The named uses are facial fat loss in people with HIV (3 August 2004), shallow to deep nasolabial fold contour deficiencies and other facial wrinkles suited to a deep dermal cross-hatch technique (2009), and fine lines and wrinkles in the cheek region (2023). The under-eye area is not among them.

Common questions

Questions Patients Ask

Is Ez Gel FDA-approved?

No. There is no FDA-approved or cleared injectable filler product by that name. Because the material is made from the patient's own blood during the appointment, there is no manufactured product for the FDA to approve. A centrifuge or kit may hold its own clearance for preparing blood components, but that is not approval of a cosmetic treatment.

What is Ez Gel made of?

Your own blood. It is drawn and spun to produce liquid platelet-rich fibrin and a plasma layer rich in albumin. The albumin layer is heated, which denatures it into a gel, and the two are recombined into something injectable. The published protocol describes roughly a 3.5 to 1.5 cc ratio of denatured albumin to liquid fibrin.

Does Ez Gel last longer than ordinary PRF?

That is the claim, and the evidence for it is animal evidence. In a mouse study, standard platelet-rich fibrin membranes were substantially or completely resorbed by 21 days while the albumin version stayed volume-stable. A review reports that heating extends resorption to at least four months in a subcutaneous animal model. That is a resorption timescale in animals, not a measured cosmetic duration in people.

Is it safer than Sculptra because it is my own blood?

It removes one category of risk, not the most serious one. Nothing synthetic is left behind, so there is no polymer for a late granuloma to form around. But the main danger of any facial injection is material entering an artery, and that risk comes from the needle and the anatomy. Neither product has an antidote.

Which one works better?

No trial has compared them, so there is no answer. Sculptra has randomized, evaluator-blinded trials that the FDA reviewed, with results measured on validated wrinkle scales past two years. The aesthetic evidence for the albumin gel is a technique article with prospective observations, written by an author with a commercial interest in the kits.

Can Sculptra be reversed?

No. Hyaluronidase works only on hyaluronic acid and has no effect on poly-L-lactic acid. No reversal agent is approved and the labelling does not mention one. The material breaks down on its own over roughly two years, and a lump is managed with saline infiltration, steroid injection, antibiotics if a biofilm is suspected, or surgery in stubborn cases.

Can either be used under the eyes?

The under-eye area is not a named indication in any of Sculptra's US approvals, and Sculptra is labelled for injection into the deep dermis with a threading technique. The albumin gel is most often described for exactly that area, in a technique paper that positions it for people wanting a conservative approach - but that is expert opinion rather than trial evidence.

How many sessions does each need?

Sculptra's trials used a course: up to four sessions about three weeks apart in the 2009 study and an average of 3.6 sessions in the 2023 cheek study. There is no published session protocol for the albumin gel, because there is no trial that set one.
